keep dying from glitch and loosing stuff in city area

Why do i keep dying randomly i just teleport randomly and just fall and die i only see blackness and bedrock is anyone else having this glitch? When this first happened i had my best stuff and lost it all I'm worried if i carry any stuff that is important or anything i will loose it all may you fix this glitch its is very annoying

Again it happened 12/2 i was just building in city area then i just fell through the ground and died lost all my stuff again i tried /back my stuff was gone i lost everything i had on me

Once again i died an lost everything on me from this glitch everything i carry i think might as well get lost where ever it goes. Also i lost about 100k worth of items from these deaths. I would just like a refund on my multi tool V
